OCD has nothing to do with perfectionism or a preference for cleanliness. It is a serious, biologically driven condition in which the brain becomes trapped in a loop of unwanted intrusive thoughts (obsessions) and repetitive behaviors or mental rituals (compulsions) carried out to ease the distress those thoughts create. According to the International OCD Foundation, OCD affects approximately 2 to 3 million adults in the US, making it one of the more common and debilitating mental health conditions.

What makes OCD particularly difficult to live with is that compulsions offer only temporary relief while quietly strengthening the obsessions over time. Many people spend hours each day caught in rituals and intrusive thoughts, often in silence, carrying shame about thoughts they never chose and would never act on.

OCD is also one of the most misunderstood and misdiagnosed conditions in psychiatry. It does not always look like washing hands or checking locks. TYPES WE TREAT

Classic OCD

Recurrent, unwanted intrusive thoughts paired with compulsive behaviors or mental acts performed to neutralize distress. Common themes include contamination, symmetry, harm, and checking.

Pure O (Primarily Obsessional OCD)

OCD in which compulsions are largely invisible mental rituals rather than physical behaviors. Intrusive thoughts about harm, religion, sexuality, or relationships with no outward compulsions to identify.

Relationship OCD (ROCD)

Obsessive doubts and intrusive thoughts focused on romantic relationships, including constant questioning of feelings, partner suitability, or fidelity, causing significant distress and avoidance.

Scrupulosity (Religious OCD)

Obsessive fears of sinning, offending God, or being morally impure, often accompanied by excessive prayer, confession, or reassurance-seeking. Frequently misunderstood as a spiritual rather than psychiatric issue.

Hoarding Disorder

Persistent difficulty discarding possessions regardless of their value, resulting in significant clutter and distress. Recognized as a distinct but related condition on the OC spectrum.

Body Dysmorphic Disorder (BDD)

Preoccupation with a perceived flaw in physical appearance that is not observable or appears minor to others, leading to compulsive checking, avoidance, and significant impairment.

Trichotillomania & Excoriation

Repetitive hair-pulling (trichotillomania) or skin-picking (excoriation disorder) that causes distress or functional impairment. Often driven by anxiety and tension relief, similar to OCD compulsions.

OCD with Co-occurring Conditions

OCD frequently co-occurs with depression, anxiety, ADHD, and tic disorders. Treating only one condition while missing the others limits recovery. Carolyne evaluates and treats the full picture.

Carolyne reviews your full psychiatric, medical, and personal history. For OCD, this includes a detailed exploration of your specific obsession and compulsion themes, onset, severity, and how symptoms are affecting your daily life and relationships.

We look beyond the OCD cycle at factors that may be amplifying symptoms, including stress, sleep, hormones, inflammation, and co-occurring conditions like depression or anxiety that often accompany OCD.

Your plan may include SSRI or SNRI medication, referral to an ERP-trained therapist, and integrative lifestyle strategies. Pharmacogenetic testing is available to guide medication selection and reduce trial and error.

Regular follow-up appointments to monitor your response, adjust medication as needed, and coordinate with your therapy team to ensure all parts of your care are working together.
